Legal & Telehealth Disclosures

Medical Emergencies

Spectrum Primary Care does not provide emergency services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use this website, our contact form, email, or telehealth visits for emergencies.

Website Information Is Not Medical Advice

The content on this website is for general informational and educational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. Using this website, submitting a contact form, or sending an email does not create a provider–patient relationship. A provider–patient relationship is established only after you complete a visit with our provider.

Telehealth Services

  • Spectrum Primary Care currently provides care through telehealth only. Services are available only to patients who are physically located in Florida at the time of their visit. (New York services are coming soon and will be offered only once licensure is in place.)

  • Telehealth has limitations, including the absence of an in-person physical examination. Your provider may determine that your concern is not appropriate for telehealth and may recommend in-person care, urgent care, an emergency room, laboratory testing, imaging, or a specialist.

  • Telehealth relies on electronic communication. Despite reasonable safeguards, technical problems or interruptions can occur. You will be asked to review and consent to telehealth services before your first visit.

  • For your privacy, please join visits from a private location using a secure internet connection.

Provider Information

Clinical services are provided by Kensy Chavez, DNP, APRN, FNP-C, a board-certified family nurse practitioner licensed as an Advanced Practice Registered Nurse in the State of Florida. A nurse practitioner is not a physician. The title “DNP” refers to a Doctor of Nursing Practice degree.

Prescriptions

Prescriptions are issued only when clinically appropriate, based on the provider’s professional judgment and in accordance with Florida and federal law. A visit does not guarantee that any particular medication will be prescribed.

Insurance and Billing

  • Spectrum Primary Care is in-network with Curative and Oscar Health for primary care telehealth visits. Coverage, copays, coinsurance, and deductibles are determined by your plan. Please confirm your benefits with your insurer before your visit.

  • You are responsible for any amounts your plan does not cover, including copays, deductibles, and non-covered services.

  • Self-pay prices listed on this website apply to visits not billed to insurance. Laboratory tests, imaging, medications, and services by other providers are billed separately by those providers unless stated otherwise.

Good Faith Estimate

Under the federal No Surprises Act, if you do not have health insurance or are not using insurance, you have the right to receive a Good Faith Estimate of the expected charges for your care. You can ask us for one before scheduling a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. For more information, visit www.cms.gov/nosurprises.

Direct Primary Care Memberships

Direct Primary Care memberships, where offered, are not health insurance and do not replace health insurance coverage. Membership fees cover only the services described in the membership agreement. Patients are encouraged to maintain health insurance for services outside of primary care, such as hospital, specialty, and emergency care.

Non-Discrimination and Language Assistance

Spectrum Primary Care complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, sex, sexual orientation, or gender identity. Care is available in English and Spanish. Atendemos en español.
